Case files, 1978-2008, containing documents relating to administrative hearings held by the state Division of Hearings and Appeals on enforcement actions by the Department of Health and Social Services under Section 50.04(4) and (5), Wis. Stats. These cases involve nursing home statements of deficiency and/or assessments of forfeiture. Included are briefs, transcripts, depositions, notes, and other legal papers. Some files contain patient treatment records entered as evidence and other files contain materials generated when the administrative decision of the division was appealed to the court system. Records pertaining to appeals for which no hearing was held by the division are not retained.
