In two parts. Part 2, "Instructions for the surveyors of the late arch-bishops and bishops lands, which are to be surveyed", has a caption title; pagination and register are continuous; it is identified in Thomason collection as E.362[16].
McAlpin Catalogue reports two impressions, one as above, the other with the words "Corrected according to the originall" inserted between title and order to print.
