Summary
Across the top in caps is the caption "Klaw and Erlanger's stupendous production," under which follows "Gen'l Lew Wallace's." Below that is the title, with "Dramatized by Wm. Young" below it and to the right. The image shows two figures staring into the heavens, over the bottom caption which reads: "'I would have thee believe, even as I believe, tho His Kingdom, be not of this earth, His Power is from Above.' Ben-Hur Act VI, Scene 4."
The play was eventually performed by scores of touring companies, so it is somewhat speculative that these all belong to the original run. By 1916, film versions of the story began to displace the theatrical versions, and this adaptation formed the basis for two of those. Associated with the Manhattan Theatre, New York.
