First phase records, 1960-1977, of the Patient Evaluation Review Committee (PERC), which was formed in the attempt to correct deficiencies in Wisconsin's nursing homes. PERC used two tools, Areas of Care Evaluation (ACE) and Time Unit Base (TUB), to evaluate patients in nursing homes and correlate patient profiles with services appropriate for individual patients. Records include ACE, TUB and other evaluation/inventory forms, meeting minutes, and reports and project proposals; correspondence, articles related to diagnostic testing and evaluation of nursing home patients, and administrative program records.
